New tools and requirements can be complex to navigate. Our goal is to make this work easier and more effective for your county. To support that, we offer one-on-one introductory sessions designed to help you understand how the Knowledge Center tools can be applied in your local context. In this session, counties will:

  • See how the Knowledge Center tools can fit your county’s priorities and capacity 

  • Learn how counties are effectively applying these tools 

  • Identify which resources are most relevant to your county’s BHSA priorities 

  • Get connected to upcoming hands‑on support: workshops, webinars, office hours, and more

Developing a Performance Management Dashboard

Create a dashboard that clearly shows how your program is performing month-to-month and how an individual experiences, engagement, and outcomes are trending. This helps identify strengths, challenges, and opportunities for improvement in service delivery and operations

Go through Resource 1A (Building a Theory of Change (TOC)) exercise from Module One to determine the core goals of your program.
 
Your TOC will be the backbone of your dashboard.
 
Select Key Performance Indicators (KPIs) using Resource 1D that represent each step of your program’s journey. Choose KPIs that:
 
  • Directly connect to TOC elements
  • Track the progression of individuals and show successes and exceptions
  • Include the experiences of individuals in the program
  • Can be regularly reported using existing systems (EHR, provider reports).
  • Help understand performance and make decisions.

Tip: Discuss which KPIs would be best to track with key frontline staff

 
Use Resource 2A.2 (Building a Performance Management Dashboard XLS) to generate monthly visualizations of your selected KPIs. Some recommended dashboard sections include:
 
  • Outreach and Engagement: Outreach attempts, Referral Type and #, etc.
  • Service Delivery and Census: Program census, staffing service engagement, etc.
  • Progress and Outcomes: Service connections progress, outcomes, etc.
  • Exits: Exit types, exit outcomes, etc.
 
Tip: Use Resources 2B and 2E to help guide your PM strategy and timeline for discussions.
